A Clinical Study of Direct Current Electrotherapy in Hemorrhoids


Abstract
Many techniques are currently employed for outpatient treatment of hemorrhoids. Sclerotherapy, Rubber band ligation, Infrared photocoagulation, and Cryosugery have been well known techniques for outpatient treatment of hemorrhoids, but
electricity
has
been introduced in korea.
The author treated 510 cases of homorrhoids by usnig lower voltage direct current generating equipment from April 1990 to Apirl 1992. 318 cases of internal (264 cases) and mixed(48 cases) hemorrhoids were treated only with direct current therapy;
but,
192 cases were treated with both direct current therapy and outpatient surgical procedures due to other anal diseases.
@ES The results are as follows:
@EN 1) Male to female ratio was 2.1 : 1. The age distribution indicated a predominant incidence in 3rd to 4th decades(57.6%) (20-50 year, 91%).
2) For the most parts(77.2% or 394 cases), the duration of symptoms was confined to a ten year period: but, 86 cases(16.8%) spanned ten to twenty years and 30 cases(6.8%) were over 20 years.
3) The main clinical symptoms of hemorrhoids were protruded mass, 94.9%(484 cases), bleeding, 66.4%(311 cases), pain, 61.4% (316 cases), and pruritus, 7.6%(89 cases).
4) The most frequent grade of internal hemorrhoids was grade 2(60.7%, 310 cases), followed grade 3(20.5%, 105 cases), grade 4(12.9% 66 cases) and grade 1(5.6%, 29 cases).
5) 318 cases of internal(264 cases) and mixed(48 cases) hemorrhoids were treated successfully using only direct current therapy, but 192 cases of internal and mixed hemorrhoids were treated with both direct current and operation of outpatient
procedure
due to other combined anal disease such as external hemorrhoids(11 cases), some mixed hemorrhoids(25 cases), thrombus formation(58 cases), fissures(46 cases), fistulae(10 cases), skin tags(36 cases), and perianal abscesses(5 cases).
6) Most of the internal and part of the mixed hemorrhoids(498 cases, 97.6%) treated with direct current therapy had good results, but a few patients(11 cases, 2.2%) were retreated with direct current for remaining protruded mass and one patient
was
treated 3 times with direct current therapy.
7) In conclusion, direct current electrotherapy is safe, easy to apply, needs no anesthesia, no bowel preparation, and also can apply to all grades of internal and a part of mixed hemorrhoids. The effect of the treatment, no matter the grade, is
excellent. The success ate is over 90%. The side effect is minimal compared to other non-operative treatments. Therefore, the author believes that direct current electrotherapy is a desirable non-operative treatment for internal and mixed
hemorrhoids.
